This health history is correct and complete to the best of my knowledge, and the person herein described has permission to engage in all event activities.

I hereby give permission to the medical personnel selected by Concordia Seminary to order X-rays, routine tests, treatment; to release any records necessary for insurance purposes; and to provide or arrange necessary related transportation for my child. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by Concordia Seminary to secure and administer treatment, including hospitalization, for the person named above. This completed online form may be photocopied for trips off the Seminary campus.

Vocatio Participant Conduct Agreement

We are delighted that your child will be a participant at Vocatio here at Concordia Seminary, St. Louis.

This is intended to explain to you and your child the standard of conduct we require on our campus. By understanding expectations in advance, we anticipate a trouble-free, uplifting time for your child at this event. If there is a problem, you and your child know in advance how this will be handled.

1. Concordia Seminary forbids participants to be in possession of alcohol, drugs, tobacco or illegal substances no matter if they are or are not being used or consumed. Possession of any of these substances will result in a parent or guardian being called immediately and the student returning home immediately at the parent's or guardian’s expense.

2. Concordia Seminary has a zero-tolerance policy on visiting the opposite gender’s rooms. Males are not permitted in any female’s (participant or Seminary student) room. Females are not permitted in any male’s (participant or Seminary student) room. Participants violating this policy will have a parent or guardian called immediately and the student will return home immediately at the parent's or guardian’s expense.

3. Concordia Seminary does not permit participants to have inappropriate physical contact with another participant or student at Concordia Seminary. This includes kissing, touching beyond acceptable social contact (i.e., shaking hands, pat on the back, etc.). Participants violating this policy will have their parents or guardians called immediately. If the infraction is deemed serious and worthy of being sent home, the student will return home immediately at the parent's or guardian’s expense.

4. Lights out – Staying in Concordia Seminary's dormitories, having a roommate and anticipating a full schedule the next day, require participants to abide by the scheduled time for lights out.

We have read the Participant Conduct Agreement addressed to parents of Vocatio participants concerning participant conduct and have shared the conduct expectations contained here with my child (named above). I agree to comply with the consequences expressed here should my child’s conduct necessitate disciplinary action during the event (even if it means changing return travel plans at my expense).

Consent and Liability Waiver

I/We understand that Vocatio, for which this Consent and Liability Waiver Form is being given could include, but is not limited to the following:

Students will be transported to various off-campus locations via charter bus, rented vans or personal vehicles of staff members to visit various St. Louis tourist sites, participating in Bible study and research sessions, participating in outdoor sports and activities, and participating in a servant event.

I hereby consent to the participation of my child in event named above. I understand that I have a duty to provide primary accident and medical insurance for my child, and I declare that my child is covered by primary accident and medical insurance.

I RELEASE AND FOREVER DISCHARGE, CONCORDIA SEMINARY, THE LUTHERAN CHURCH-MISSOURI SYNOD, THE GROUP LEADERS, DIRECTORS, TRUSTEES, OFFICERS, EMPLOYEES AND OTHER REPRESENTATIVES FROM ANY AND ALL DAMAGES AND CAUSES OF ACTION EITHER AT LAW OR IN EQUITY THAT I MAY HAVE AS A RESULT OF MY CHILD’S PARTICIPATION IN, ATTENDANCE AT, AND TRAVEL TO AND FROM THE EVENT. FURTHERMORE, I DO HEREBY EXPRESSLY STIPULATE, AND AGREE TO INDEMNIFY AND HOLD FOREVER HARMLESS CONCORDIA SEMINARY, THE SYNOD, THE GROUP LEADERS, DIRECTORS, TRUSTEES, OFFICERS, EMPLOYEES, AND OTHER REPRESENTATIVES AGAINST LOSS FROM ANY AND ALL PRESENT OR FUTURE CLAIMS, DEMANDS OR ACTIONS IN LAW OR IN EQUITY THAT MAY HEREAFTER BE MADE OR BROUGHT BY MY CHILD, OR BY ANYONE ELSE ON THEIR OWN BEHALF FOR DAMAGES OR ANY OTHER LEGAL OR EQUITABLE REMEDY ON ACCOUNT OF ANY INJURY, ILLNESS, PHYSICAL CONDITION, INCONVENIENCE OR LOSS SUSTAINED BY MY CHILD DURING THE EVENT OR TRAVEL TO AND FROM THE SAME.
